The headlines read “Priligy stops premature ejaculation” and “A new treatment for men with premature ejaculation”. Both these reviews paint a critical but divergent picture, and they fail to convey any insight into the impact of Priligy (dapoxetine) on premature ejaculation (PE).
First, there is the “”. It refers to the prescription drug dapoxetine (), which has been on the market since 1998, in which case it is still available in many strengths. However, in 2012, after an extensive review and adjustment in the way Priligy was prescribed, dapoxetine became available as a branded tablet. This led to significant savings for patients and their partners. A 2013 review found that dapoxetine was associated with a “significant increase” in the time it took to ejaculate, as compared to a placebo. In 2019, a similar review found that dapoxetine was associated with a “significant reduction” of ejaculation time. One other review found that dapoxetine was associated with a “significant increase” of time to ejaculation, as compared to a “placebo”. These reviews have failed to provide any insight into the efficacy or safety of the drug.
Second, there is the “off-label” “approach” approach. This refers to the use of drugs in an off-label manner, which has resulted in some patients being prescribed “dapoxetine” instead. The decision to use an off-label manner is complicated, and there is no “on-label” approach. Rather, the patient is treated in a different setting, and the off-label manner is used for the patient’s own benefit. However, these two approaches are complementary and do not create a uniform treatment picture.
In 2018, a similar review by Pfizer published a paper suggesting that dapoxetine was associated with a “significant reduction” of ejaculation time. This review was updated to include information from the literature, but the findings did not change the way Priligy was prescribed. Rather, the review concluded that dapoxetine was associated with a “significant reduction” of time to ejaculation, and was not associated with an increase in ejaculation time. As a result, the paper concludes that dapoxetine is not an effective treatment for premature ejaculation, and has “no long-term safety profile”.
Another review, published in 2019, by the Cochrane Collaboration, found no evidence that dapoxetine is associated with an increase in the time to ejaculation, but that it is associated with a “prevalence” of PE. The review concluded that dapoxetine is not an effective treatment for PE, but that the evidence is insufficient to recommend it for long-term use. These reviews have failed to address the complexities of this process, and the decision to use Priligy (dapoxetine) is “a matter of personal preference”.

Dr Claire O’Connor, an endocrinologist, toldThe Guardianthat she used to be prescribed dapoxetine, and that Priligy was the first drug approved “off-label” for the treatment of premature ejaculation. However, she said that the evidence to support its use for this indication is weak, and that it has a “long-term safety profile”. She added that the use of this drug “is still considered an off-label use, but there are limited clinical data to support its use, and there is no long-term safety profile”.
Dr Catherine Ross, a sexual health specialist, toldthat although the evidence to support the off-label use of dapoxetine for this indication is weak, she has seen no clinical evidence to support its use for this indication.
